    Photographic Images & Names - Consent Form

    To comply with Data Protection Law, we need your permission to photograph or make any recording of your child.

    The table below shows the different ways your child’s image/name may be used.

    PLEASE TICK TO CONFIRM YOUR CONSENT OR OTHERWISE FOR EACH MEDIUM.
    A SEPARATE FORM MUST BE SUBMITTED FOR EACH CHILD YOU MAY HAVE AT OUR SCHOOL.

    INTERNAL DISPLAYS
    How: Your child's image and any names features will be first name and initial of surname by default.
    Where: in school
    Examples: displays, learning resources, end of year class photos, school plays and productions recorded and streamed to parents, school play programmes

    How: Your child's image. The school will not feature the child's name in any format, unless you have specifically consented to it.
    Where: Trust or school publications
    Examples: Trust or school newsletter, Trust or school website, Trust or school approved apps eg Tapestry, SeeSaw & Scratch Junior

    Declaration

    • I have read and understood the consent asked of me above.

    • I understand that I will be given the opportunity to review my consent choices at the end of Year 2.

    • I understand that I may change my consent status at amy time by notifying the school in writing to office.waltonoak@lumenlearningtrust.co.uk or Office Manager, Walton Oak Primary School, Ambleside Avenue, Walton-on-Thames KT12 3LN.

    • Consents given apply for up to a maximum of 5 years after my child has left the school.

    • I promise that if I, or members of my family, take photographs or video recordings at a school event, these will be kept for family use only.

    • I, or members of my family, will not publish any material online that features an image of any child other than my own.
